Please complete this application to request access to the CHEMPACK SharePoint Site. All applications are submitted to the CHEMPACK Administrator for approval. If your application is approved you will be contacted and given your login information. (* required)

Please note: Access to the CHEMPACK Site requires Microsoft 365 authentication. Most email accounts are associated with Microsoft 365 or can easily be associated. In rare cases, some email accounts cannot be used, in this case we will help you find an alternative.
